Life Certificate

Type of Document

An individual who needs a Life Certificate, for the purpose of drawing pension in India may apply for this service (if an individual is required to produce Life Certificate before authorities in India for any other purpose)

How to apply

The application for the Life certificate may be submitted at the Embassy of India, Buitenrutweg 2, 2517KD The Hague, on a working day between 9.30 and 12.30

Miscellaneous Application Form dully filled in and signed by the applicant.

Documents Required

1. In case of Indian Nationals: Valid passport in original and self-attested photocopies of first two and last two pages of the passport

2. In case of Foreign Nationals: Valid passport in original and self-attested photocopies of the passport pages containing personal particulars.

3. Proof of Residence: Any of the following self-attested documents viz. copy of utility bill (phone bill/electricity bill/gas bill/water bill) or lease deed (of house) etc.

4. Copy of passport page with visa/Dutch Residence Permit Card of the applicant showing the visa status should be enclosed.

5. Applicants are requested to bring the dully filled life certificate along with the application for the embassy to attest it. The format for life certificates are provided by the bank/ life certificate receiving organization/ pension provider. Please check the website of the bank/ life certificate receiving organization/ pension provider for the life certificate format.

Processing Time

Normally, Same Day/Next Working Day subject to documentation being in order.

Fees

Gratis for pension purpose. For other purposes Euro 21( including euro 2 for ICWF)

Note: Apart from the general mandatory requirements mentioned above, applicant may be advised to submit additional documents, after scrutiny, on case to case basis.
