Type of Document

Birth Certificates are issued to Indian passport holders and Identity Certificate(IC) Holders based on passport and IC. Applicants should mention the purpose for which this birth certificate will be used.

How to apply

i)  Fill up the form online Miscellaneous form

ii)Take a printout of the online filled in form. The form should be duly signed with photographs and necessary documents (please visit check list for documentation).

iii) Book an appointment (link)

iv)Book prepaid airway bill with POST/Courier service in the Netherlands for receiving processed documents. This should be submitted along with an A4 size self-addressed envelope 

v)  Submit application for Passport alongwith necessary documents and prepaid A4 size self-addressed envelope at Embassy of India in The Hague (Buitenrustweg 2, 2517KD, The Hague

Documents required

A duly filled and signed Miscellaneous application form (see download forms section on our website)

2 One photograph conforming to Indian Government norms as per specifications mentioned on our website (5 cm * 5 cm)

3 Self-attested photocopy of Indian passport (first page, last page) and original for verification / self-attested photocopy of Identity Certificate and original for verification

4 Supporting documents to show the reason for requesting the birth certificate 

5. Self-explanation letter clearly stating the purpose for which this certificate is required and reason for non-availability of Indian Birth certificate

6.Self-attested photocopy of the Dutch residence permit (front and back) and original for verification 

7. A prepaid A4 size self-addressed envelope

Fees

€10.00 (payable by debit card (PIN). Cash is accepted as an exception if exact amount is tendered).

Processing Time

Minimum 4-6 working days from date of receipt of application at the Embassy.
